Umpqua Health’s Transition of Care Team is here to see your recently discharged ATRIO DUAL and UHA patients. Our team will work to ensure the health of patients at risk for readmission.

Umpqua Health enrolls members in the post-acute care Transition of Care (TOC) program. The program helps bridge the gap between hospital, home, and follow-up care with primary care providers. Additionally, the TOC team offers health coaching to assist patients in gaining confidence in managing their medical needs. The coaching program includes one face to face visit, medication reconciliation, chronic disease management education, and referrals to community resources for identified needs after hospital discharge. The goal is to help patients make the transition from the hospital back to home and guide further medical follow-up.
