Umpqua Health Alliance Partners with Connect Oregon to Connect Residents to Health and Social Resources
Umpqua Health Alliance (UHA) and Unite Us, the enterprise technology company providing the digital infrastructure to power integrated health and social care, are partnering to expand the coordinated care network known as Connect Oregon. This partnership will strengthen a commitment to create healthier communities and enhance the quality of life for Oregon residents most in need. The Connect Oregon network establishes a single space where UHA can connect patients to local health, human, and community-based organizations in Douglas County and across the state, to address and improve whole-person care across sectors.
The Connect Oregon network enables health and social care providers to connect people to much-needed resources and services such as assistance with housing, behavioral health resources, social services, food insecurity, and employment assistance. Community-based organizations that provide local social services can join Connect Oregon at no cost. Through Unite Us’ shared technology platform, healthcare, social care, and community partners can identify needs, refer individuals to resources and track outcomes through a secure, closed-loop coordinated care network.
Brent Eichman, Umpqua Health Alliance’s CEO, acknowledges the importance of elevating social care to the same priority as healthcare to improve the community’s overall health.
“There are many aspects of a person’s health that social determinants of health (SDoH) conditions impact, which are the places where people are born, live, work, and play,” said Eichman. “These conditions may affect a person’s health risks and outcomes. With this partnership, the Douglas County community can prioritize social services and will meet people’s needs outside of the four walls of the doctor’s office,” he added.
Together Umpqua Health Alliance and Unite Us will securely connect people in need with community-based resources that can be difficult to navigate. The network provides a central point of contact where health care providers, social service organizations, and individuals can securely access and refer people to needed services while monitoring progress and measuring outcomes.
“Our collaboration with Umpqua Health Alliance will help us reach a collective goal to increase equitable access to healthcare and social services for residents, regardless of circumstance,” said Celia Higueras, Oregon State Network Director, Unite Us. Together, these critical resources and services will redefine the quality, impact, and intersection of whole-person care across Douglas County and the state,” Higueras added.
